My athlete(s) participating in this program are included in a medical insurance policy that I maintain. I hereby acknowledge this release from liability for accidental injury or illness which my athlete(s) may incur as a result of participating in any physical activity. I hereby assume all risks connected therewith and give my athlete(s) full consent to participate in this program. I agree to disclose any physical limitations, disabilities, ailments, or impairments which may affect my athlete(s) ability to participate in this program.
